Initial labs are included. Subsequent labs and medications are not included in the program fee. Where possible, we'll route labs through your insurance and recommend the most cost-effective options for any prescriptions. Supplements are not included but members receive 35% off through Fullscript.

Using insurance?
Most PPO patients recover $1,000 to $1,500.
We're out-of-network, and Washington PPO plans typically reimburse 40% to 60% of program costs through out-of-network benefits. We've partnered with Sheer Health to make reimbursement submission easy, with no paperwork on your end.

At the end of our 16 weeks together, we can facilitate a transfer of care to your primary team, or continue with our quarterly maintenance plan, the Phase Forward Club.

Payment & Billing
Payment
This program is billed monthly at $1,200 for month 1 and $800/month for months 2 to 4 ($3,600 total). By enrolling, you authorize Phases Clinic to charge your card on file automatically on a monthly basis for the duration of the program.
Commitment
This is a 4-month program commitment, not a month-to-month subscription. Enrollment constitutes agreement to the full program term regardless of attendance.
First-month payment
The $1,200 first-month payment reflects the new patient evaluation, initial lab work, and extended session time incurred at the outset of the program. This amount is non-refundable after enrollment as these costs are incurred immediately upon intake.
Missed appointments
Missed or canceled sessions do not pause or reduce billing obligations. Your slot is reserved and clinical time is allocated regardless of attendance.
Nonpayment
Failure to maintain a valid payment method or nonpayment of any installment may result in forfeiture of remaining scheduled visits until the account is brought current.
Non-refundable
All payments are non-refundable, except in cases where the clinic is unable to provide the scheduled services.
HSA / FSA
This program is HSA/FSA eligible. Superbills are provided upon request for out-of-network insurance reimbursement. Reimbursement is not guaranteed and is the patient's responsibility to pursue with their insurer.
What's included & excluded
Labs
Initial lab work is included in the program fee. Subsequent labs are not included. Where possible, we will route labs through your insurance and recommend the most cost-effective options.
Supplements & medications
Supplements and medications are not included in the program fee. We will recommend the most cost-effective insurance-based or cash-pay options available.
Health coaching
Monthly health coaching sessions are delivered by an independent contractor. Phases Clinic coordinates this service as part of the program.
Scheduling Policy
Cancellations
At least 24 hours' notice is required for any cancellation. Sessions canceled with less than 24 hours' notice will be forfeited.
General Agreement
Availability
This program is exclusively available to residents of Washington State.
